This whole album is about a man falling in love with a girl who is terminally ill. The pain in this song refers to her cancer, read http://songmeanings.com/songs/view/3530822107858757468/

How can a song be so perfect? how can an album be so perfect? the whole thing is an analogy to my life, but in an organic sense, because its meaning twists and turns, as my life twists and turns....

this track is about Peter Silvermans late wife who had no choice but to abort her unborn child after she found out it was terminally ill. Hospice is about a period of about 15 years where Peter lost both his sister and his wife to cancer and later went into a deep depression. This album is so damn powerful it makes me tear up every time i listen :(

Definitely about abortion, come on guys. "The cub's been kicking from within"? "Drink champagne although you shouldn't be"? "We're not scared of making caves or finding food for him to eat"? I know the album is predominantly about cancer but this song clearly isn't.
